Unfortunately, I cannot reproduce with either QCAD Professional or QCAD/CAM on Windows 10, 64bit.

It's certainly possible through the script interface.
There's no GUI to somehow click together your own toolbar.

This has been implemented and will be included in the next snapshot release and possibly in the next stable release of QCAD Professional.

I'd do a clean installation from scratch (no need to remove anything).

You can type any value into this box or change the scale later using the property editor (View > Property Editor, select viewport, change property "Scale".

Which package did you download? What's your distribution version? 32bit or 64bit? The function include does not return anything. So that result of calling include is undefined. It's a JavaScript oddity but doesn't mean it didn't work.
